Naomi McLoughlin represented a nurse in connection with the death of an elderly patient at Barnsley Hospital. The deceased, who suffered with a number of significant comorbidities, had erroneously been given a 200mg dose of morphine instead of the prescribed  20mg dose. Naomi represented the nurse who administered the dose  with the nurse facing potential serious professional consequences arising out of any findings made in the Inquest.

The Coroner was satisfied that the error was not singularly attributable to the nurse but that there had been a series of opportunities to prevent the overdose which were missed by others. He returned a narrative conclusion and in particular found that the handwriting on the prescription was ambiguous.
